The private medical villa outside Fiesole sat perched high above the Arno Valley, surrounded by towering cypress trees and guarded by eight armed Bellini operatives flown in on emergency charters from Milan.

Inside the master suite on the second floor, the room was bathed in the warm, flickering amber light of a massive stone hearth. The air smelled of woodsmoke, antiseptic, and vintage Chianti.

Matteo sat propped up against a mountain of pillows on the king-sized bed, his broad chest bare, wrapped in thick white surgical bandages that immobilized his left shoulder. The local surgeon had successfully extracted the bullet two hours earlier, repairing the torn deltoid muscle and confirming no major arteries had been severed.

Elena sat on the edge of the mattress, wearing a borrowed oversized sweater, holding a cool linen cloth against his sweating forehead. Her hands were still faintly trembling, but her dark eyes were entirely clear.

"The doctor said you have the constitution of an ox," Elena said quietly, her voice tinged with lingering adrenaline. "An inch to the right, Matteo, and that round would have severed your subclavian artery. You would have bled to death in that alley before your men even cleared the street."

Matteo looked up at her, a faint, wry smile touching his pale lips. "Then it’s fortunate my aim was good."

Elena set the cloth down into a ceramic bowl on the nightstand. She looked at the scars crisscrossing his muscular chest—remnants of past betrayals, past wars, a life lived constantly on the precipice of slaughter.

"Why did you do it, Matteo?" she asked softly, her gaze searching his. "You are the head of the Bellini syndicate. You have three thousand men who would take a bullet for you. Why throw yourself in front of a gun for a woman who ran away from your altar?"

Matteo reached out with his uninjured right hand, his warm, heavy fingers wrapping gently around her wrist, pulling her closer until her hand rested directly over his heart.

"Because three thousand men can be replaced with money, Elena," Matteo answered, his voice dropping into a raw, gravelly register that made her breath catch. "You cannot. When I walked into that empty bridal suite in Chicago, I realized that for twenty-six years, I have been building an empire out of corpses and gold. I thought power was commanding respect through fear. But when you left that note, I felt a terror that no rival family, no federal prosecutor, and no bullet could ever inflict upon me."

He pulled her hand up, pressing his lips to her knuckles with breathtaking reverence.

"I realized that without you, the empire is just a cage," he continued, a tear slipping from his eye, cutting through the stubble on his cheek. "I don't care about the Chicago docks. I don't care about the Moretti alliance. If you tell me to walk away from the syndicate tonight, I will sign the operational authority over to Marco and spend the rest of my days carrying your easel through the hills of Tuscany."

Elena stared at him, feeling the heavy, undeniable truth vibrating in his chest.

"And what about the Morettis?" she whispered. "Antonio Moretti sent hitmen to murder you in a foreign city."

Matteo’s eyes darkened, the lethal, ancient fire of his bloodline igniting once more. "Marco traced the contract sixty minutes ago. Antonio Moretti didn't act alone. My mother... Lucia... authorized the communication channel. She believed that if you were eliminated, I would have no choice but to return to Chicago and marry Francesca."

Elena gasped, her hand flying to her mouth in utter horror. "Your own mother?"

"My mother is a creature of the old regime," Matteo said coldly. "She believes bloodlines exist only to serve political power. She forgot that I am the Don. Not her."

Matteo reached down to the bedside table, picking up a small, weathered leather pouch tied with a raw hemp cord. He untied the knot with his right hand, tilting the pouch into his palm.

Out rolled a heavy piece of gold.

Elena’s eyes widened to saucers.

Resting in Matteo’s palm was the Bellini family ring. The true ring. The dark, ancient black diamond, cut eighty years ago, held within heavy, hand-carved gold prongs bearing the miniature eagle and anchor crest. The stone seemed to absorb the light of the fireplace, radiating a deep, cold, absolute authority.

"The ring Francesca wore at the Drake Hotel was a duplicate," Matteo revealed, his eyes locking onto hers. "My mother had a master jeweler in New York craft a replica three weeks ago to trick the Moretti family into signing the port concessions. The real ring—the ring that carries the true sovereignty of my ancestors—has been locked inside my personal safe since the day I purchased your engagement ring."

He lifted the heavy gold band between his thumb and forefinger.

"I didn't give it to you at the gala because I was an idiot, Elena," Matteo confessed, his voice breaking with genuine, agonizing remorse. "I wanted to keep you separate from the violence. I wanted you to remain pure, untouched by the blood on my family’s hands. I thought by keeping you away from the family mythology, I was protecting your soul. I didn't realize that by denying you your rightful place, I was stripping you of your dignity."

Matteo carefully swung his legs over the edge of the bed, his face tightening in pain as he ignored the agony in his stitched shoulder. He lowered his tall frame onto one knee on the polished terracotta floor, looking up at her from the ground.

"Elena Mercer," Matteo swore, his deep voice carrying the sacred weight of a blood vow. "You are not an outsider. You are not a temporary alliance. You are my queen. You are the only woman who will ever command my house, the only woman who will ever rule my life. If you will have me... take this ring. And let me spend the rest of my life proving that you made the right choice."

Elena looked down at the most feared man in Chicago, kneeling half-naked and bleeding on a stone floor, offering her not just a piece of jewelry, but his absolute, unreserved submission.

The pain, the humiliation, and the cold isolation of the past week completely dissolved, washed away by the brilliant, undeniable fire of his love.

"Matteo..." she whispered, hot tears spilling down her cheeks as a radiant, breathtaking smile broke across her face. "Stand up."

"Not until you answer me," he said stubbornly.

Elena laughed through her tears, reaching down with both hands to take his uninjured arm, pulling him gently back to his feet.

"I will take the ring, Matteo," she said, her voice ringing with absolute certainty. "On one condition."

"Name it," he said instantly. "Anything."

"We go back to Chicago," Elena declared, her eyes flashing with cold, regal steel. "And we show your mother, the Morettis, and the entire city exactly who rules the Bellini family."

Matteo’s smile was breathtaking and ferocious. He took her left hand, sliding the heavy, cold black diamond onto her ring finger.

It settled against her skin with terrifying perfection—an unbreakable crown of gold and stone.
